For your reference, proforma of specimen on-line application form (Part I and Part II) have been
uploaded separately. Refer the same before filling the application form on-line.
The processing of application form is based on the information filled by the candidate.
Any error committed intentionally or otherwise may result in cancellation of your candidature.
Part I
Generation of bank challan
1. Write your name as appearing in your 10th class mark sheet.
2. Write father’s name.
3. Write mother’s name.
4. Select your date of birth as appearing in your 10th class mark sheet, only from the
calendar appearing on the screen.
5. Select gender: Male/Female
6. Select any one category as applicable to you: General / SC / ST / OBC /SBC
7. Select any one sub-category as applicable to you:
Not handicapped physically (when you are not physically handicapped i.e. normal)/
PH (physically handicapped) /
PH with bone deformity OL & OA
8. Select your marital status: Unmarried / Married / Divorcee / Widow / Widower
9. Select knowledge of Hindi language in Devnagri script (lipi): Yes / No
10. Present communication address:
Write the full present address along with city, pin code, district where the communication
to you could be made.
Select your State from the list given.
11. Write your residence phone number (with STD code), if available, otherwise leave blank.
12. Write your cell number (without zero ‘0’) i.e. only 10 digit number, or a cell number where
you could receive a message sent by RUHS, when required.
13. Write your email ID.
Check and verify the information filled by you before pressing the submit button.
Important: This screen is not your application form. Do not print this screen. Your Part I
of application form (that you have filled on-line) will be generated only after clicking on
‘submit’ button appearing below.
14. Press submit button.
Wherever any correction is required, the text on left hand side will appear in Red colour.
Correct the same and then click on ‘Submit’ button.
A box will appear on the screen, Are you sure, all entries are correct Ok / Cancel
If you find that any information filled by you is not correct you are required to click Cancel
button, correct the information (including your date of birth) and press submit button.
When you find that all the information filled by you is correct you may press Ok button.
15. Note down the user name and password for further steps.
Move to home page and select the option “Print bank challan”.
Enter your user name and password to print bank challan.
A separate sheet giving your user name (your application form no.) and password
alongwith the details filled by you will also be generated. Keep it safe.
16. Deposit the application fee at any branch of Oriental Bank of Commerce.
Ensure that you obtain Bank transaction ID (on all the three slips) from the branch of
Oriental Bank of Commerce where you deposit the application fee using your bank
challan.
Filling on-line application form
17. To fill your application form on-line, select the option “candidate login”
18. Enter your user name and password and press submit button.
19. Select “fill on-line application form, Part II”
20. Enter your valid bank transaction ID and date of deposit of application fee through bank
challan at the bank. Select this date only from the calendar appearing on the screen.
21. Permanent address: Write the full permanent address along with city, pin code, district.
Select your State.
22. If you are married:
Write your Husband / wife name
Marriage registration number, if available
(You will be required to produce your marriage registration certificate at the time of
recruitment, if selected).
Also enter the following details:
No. of alive children as on 01.06.2002
No. of children born and alive after 01.06.2002
Total alive children
If you are married and do not have any child, enter the number of children at all the three
places as 0 (zero).
23. Enter your academic qualification:
10+2 or an equivalent examination details
Write the name of board / university (as appearing in the mark sheet) from where you
have passed the 10+2 or an equivalent examination.
Write year of passing the 10+2 or an equivalent examination.
Eligibility degree details
Tick any one (eligibility degree details):
Degree in Pharmacy (B.Pharm)
Degree in Pharmaceutical Sciences
Degree in Medicine with specialization in Clinical Pharmacology or Microbiology
Write the name of university
Whether the university is established in India by law: Yes / No
Write year of passing
Write the total maximum marks (all years) and total marks obtained (all years)
or
Write CGPA, if applicable.
24. Check your knowledge of Hindi language in devnagri script (lipi)

Speak
25. Upload your recent passport size coloured photograph and signature in jpg, jpeg format
(maximum size 30 kb)
Check and verify the information filled by you before pressing the submit button. Press
submit button.
Important: This screen is not your application form. Do not print this screen. Your
complete application form (that you have filled on-line) will be generated only after
clicking on ‘submit’ button appearing below.
Wherever any correction is required, the text on left hand side will appear in Red colour.
Correct the same and then click on ‘Submit’ button.
26. View the completed application form that you have filled on-line (to check correctness of
the data filled.
If you find that any information filled is not correct you are required to click Back button
and click “Refill Part II of your on-line application form” option and complete Part II and
submit.
When you find that all the information filled by you is correct you may opt for printing the
application form by clicking Back button.
27. Check on the following Declaration: It is a must.
I hereby declare that all the information given by me in support of my application
is true, complete and correct to the best of my knowledge and nothing has been
hidden by me. If any of them is found to be incorrect or false, I shall be liable to any
legal action and / or termination from the services as may be deemed fit by the
appointing authority, Govt. of Rajasthan, Jaipur.
I have knowledge of Hindi language in devnagri script (lipi) and I can Write, Read
and Speak Hindi.
I have gone through all the rules, instructions of the notification and I promise to
abide by them.
I fulfill the prescribed eligibility criteria relating to age, educational qualification
etc. for Drug Control Officer recruitment process.
Once you check on the declaration, Part II of your application form will turn non-editable.
28. Take printout on A4 size paper by clicking ‘Print application form filled on-line’ option.
You have to print at least one copy of your filled application form.
Paste your unattested recent passport size coloured photograph, at the space provided on
the printout of application form. This photograph must be same as uploaded by you while
filling the on-line application form.
29. Write place and sign the declaration on the printout of the application form. It is a must.
30. The list of enclosures specific to your application form shall be printed at the end of the
application form. Complete the application form accordingly and keep safe for submission
when required. You are not required to send to RUHS the print copy of your application
form filled on-line.
